DESCRIPTION:Advance Care planning ensures that your values and preferences related to medical and end-of-life decisions are honored in the case that you are not able to communicate your wishes yourself. It is a process that includes choosing a healthcare agent, completing an advance care directive, and discussing your plan with those closest to you. In this one-hour session, Ozaukee County Community Nurse Educator, Kelly Barboza, BSN, RN, from Aurora HealthCare, will review the importance of the advance care planning process and encourages individuals to go on to complete advance care directives.
